- 1) I am consenting that the child/ren named above may be photographed IN A GROUP SETTING, for use in our club social media promotions, on our club website, and for general club promotional activities, with no names associated with the photograph.
- 2) I understand that the club will seek my consent for any photographs depicting my child/ren individually, for use in our club social media promotions, on our club website, and for general club promotional activities.
- 3) I understand that all formal team photographs will be published on our club website as a default, but with no names associated with the photographs.
